Apache County Office of Vital Records located at 75 W. Cleveland St., St. Johns, AZ, issues certified copies of birth and death certificates. Requests can be made in person or by mail. Cash payments are no longer accepted. Money orders and debit or credit cards may be used for both in-person and mail-in requests.

Birth and Death Certificates:  Apache County Office of Vital Records issues certified copies of birth and death certificates for births and deaths occurring In the state of Arizona only.

The office is open from 8:00 am to 4:00 pm for over-the-counter requests for certified copies. It would be advisable to call prior to coming in, especially if you are traveling a long distance.
