Call to the Public. 

1.  A/D CONSENT ITEMS: All items indicated by an asterisk (*) will be handled by a Single vote as part of the consent agenda, unless a Board Member, County Manager or member of the public objects at the time the agenda item is called. 
 
County Manager/Clerk of the Board:

*A.  Approval of Minutes dated August 6, 2012 and August 13, 2012.
 
*B.  Request approval of the demands.

*C.  District II: Request approval of a donation of $700.00 to the Ft. Defiance Senior Citizens Center for food during Elder Fest on September 6, 2012, utilizing District II funds. 

*D.  District III: Request approval of a donation of a television to the St. Johns Senior Citizens Center for their physical fitness & wellness program, not to exceed $620.00, utilizing District III funds. 

*E.  District III: Request authorization to purchase food in the amount of $2,500 for the Round Valley Senior Center utilizing District III funds.
  
Human Resources: 

*F.  District II: Request authorization to remove Joaquin Watson, Sr. from probationary status effective July 30, 2012 and Valentino Nez from probationary status effective August 14, 2011, both with the 2.5% end of probation increase.

*G.  Probation Services: Request authorization to remove Robert Talasek from probationary status with the 2.5% end of probate increase, effective July 24, 2012.

*H.  Treasurer's Office: Request approval of the job description for the Chief Deputy Treasurer to require a Bachelor's degree in accounting or finance.

*I.  Sheriff's Office: Request authorization to remove Daniel Brown from probationary status with the 2.5% end of probation increase, effective October 4, 2011.

Community Development:
 
*J.  Request approval of a conditional use permit for Marcus and Josie Forrester to construct a R.V. trailer park with 7 units. The property is located in Sanders, Arizona parcel 207-21-007.

Engineering Department:
  
*K.  Request authorization to solicit bids for a new belly dump trailer for District I, utilizing carryover funds.  

*L.  Request authorization to advertise for bid for the construction of the Tsaile Trail utilizing Transportation Enhancement Funds.

2.  A/D Engineering Department: Request approval of Amendment #4 to Intergovernmental Agreement #JPA 06-016T between Apache County and th Arizona Department of Transportation for roadway maintenance. 

3.  A/D Engineering Department: District I requests authorization to purchase a 2013 Dually 4x4 diesel Chevrolet crew cab using carryover funds utilizing State Contract not to exceed $41,427.64. 

4.  A/D Election Department: Request approval of the official Canvass of the August 28, 2012 Primary Election.

5.  A/D County Manager: Request authorization to purchase a 2012 Chevrolet Equinox for the County pool, at a not to exceed cost of $25,300.00 utilizing State Purchasing.    

6.  A/D County Manager: Request approval of the Management Agreement between Apache County and the Nutrioso Community Association to renovate and use the old Nutrioso School as a Community Center.

7.  A/D Treasurer's Office: Request authorization to convert the vacant Personal Property/Back Tax Assistant II (range 24) to an Accountant (range 49).

8.  A/D Treasurer's Office: Request approval of the Apache County Treasurer's  Investment Policy.

9.  A/D Sheriff's Office: Request authorization to accept Emergency Management Preparedness Grant in the amount of $125,185.00, administered through the Arizona Division of Emergency Management. (ADEM). 

10.  A/D Sheriff's Office: Request authorization to accept Governor's Office of Highway Safety Grant #2013-410-002 for one digital in car camera system to enhance DUI enforcement in the amount of $5,000 with no cash match required. 

11.  A/D Sheriff's Office: Request authorization to accept Governor's Office of Highway Safety Grant #2013-PT-003 for speed enforcement in the amount of $5,000 with no cash match required.

12.  A/D Sheriff's Office: Request authorization to accept Governor's Office of Highway safety Grant #2013-410-001 for one case of blood kits and overtime with ERE's to enhance DUI enforcement in the amount of $5,200 with no cash match required.

13.  A/D Notification of the Small Counties Forum on September 12, 2012 at 6:30 p.m. at the Radisson-Phoenix Airport North Hotel, 427 N. 44th Street in Phoenix and the County Supervisors Association (CSA) meeting on September 13, 2012 at 10:30 a.m. at the C.S.A. Building, 1905 W. Washington Street in Phoenix.

September 4, 2012

1.  A/D   Request approval of Intergovernmental Agreement # ADHS11-004608 amendment #2 Women, Infant and Children (WIC), Breastfeeding Peer Counseling   (BEPC), and Farmer's Market Nutrition program (FMNP) services to increase specific objectives for nutrition services.

2.  A/D  Request approval to hire a temporary Lay Health/Community Outreach Worker for 120 days (19 hours per week) at $9.75 per hour to be paid from the Public Health Assessment Grant.

3.  A/D  Request approval of the Intergovernmental Agreement ADHS03-029324 for the Well Woman Healthcheck Program.

APACHE COUNTY LIBRARY DISTRICT 
HELD IN CONJUNCTION WITH THE BOARD
OF SUPERVISORS MEETING
September 4, 2012


1.  A/D   Request authorization to accept State Grant-in-Aid in the amount of $25,000. This is an ongoing grant from the Arizona State Library.

2.  A/D Request authorization for Fiscal Year 2012-2013 to dispose of cardboard and newspapers by delivering them to Mobile Recycling USA for paper recycling. All proceeds will benefit the children's summer reading program.

3.  A/D  Request authorization for Fiscal Year 2012-2013 to make available to Arizona State Prison Complex-Winslow-Apache Unit discarded library materials and gift materials which are out of scope for our collections.

APACHE COUNTY JAIL DISTRICT 
HELD IN CONJUNCTION WITH THE BOARD
OF SUPERVISORS MEETING
September 4, 2012


1.  A/D  Sheriff's Office: Request authorization for the approval of a standard form nurse contract and its use for up to a maximum of four (4) jail nurses at any given time. Request is made to ensure sufficient coverage during periods in which permanent nursing staff is unavailable.
