NOTICE OF PUBLIC MEETING AND AGENDA OF THE
APACHE COUNTY LIBRARY DISTRICT  
HELD IN CONJUNCTION WITH THE
BOARD OF SUPERVISORS MEETING
November 18, 2014

1.  Discussion and possible approval to close District libraries on Friday, December 26th and Saturday, December 27th, and Friday January 2, 2015 and Saturday, January 3rd, 2015 due to low demand for library services on these holiday weekends.

2.  Discussion and possible approval for the St. Johns Public Library to participate in the   City of St. Johns Christmas Light Parade.


NOTICE OF PUBLIC MEETING AND AGENDA OF THE
APACHE COUNTY PUBLIC HEALTH SERVICES DISTRICT  
HELD IN CONJUNCTION WITH THE
BOARD OF SUPERVISORS MEETING
November 18, 2014

1.  Discussion and possible approval of a Memorandum of Understanding (MOU) between Apache County Public Health Services District and Window Rock Unified School District #8 to provide physical facilities to support personnel who are providing the Teen Pregnancy Prevention Services program. This is a grant funded program.

2.  Present information on an Arizona Vital Records substantive policy statement simplifying the process of obtaining a Delayed Arizona Birth Certificate for Native Americans born before 1970. Two Delayed Birth Registration Fairs will be held December 3rd and 4th, located in Holbrook and Luepp.

3.  Update on Apache County Public Health Services District's response to Ebola, including preparation, contact with partners, and communication with the State Health Department.

NOTICE OF PUBLIC MEETING AND AGENDA OF THE
APACHE COUNTY BOARD OF SUPERVISORS
November 18, 2014


1.  Notification of the Retirement of Charles Moter, Director of Probation Services, effective December 31, 2014 and request authorization to payout accumulated sick leave balance per Apache County Human Resources Policy 3.25.

2.  District III/Engineering Department: Following a Public Hearing, discussion and possible action regarding the addition of adding N3114 (located near Vernon, Arizona) to the Apache County Road Maintenance System, utilizing District III funding. 

3.  Malena Bazurto, Grants Manager: Following a Public Hearing to gather citizen input regarding Community Development Block Grant funds from the State Special Projects account, Apache County intends to apply for up to $300,000 in grant funds for use in various community improvement projects.

4.  Discussion and possible approval of CONSENT ITEMS: All items indicated by an asterisk (*) will be handled by a single vote as part of the consent agenda, unless a Board Member or the County Manager objects at the time the agenda item is called.
    
County Manager/Clerk of the Board:

*A.  Request approval of minutes dated November 4, 2014.

*B.  Request approval of demands dated November 4, 2014 - November 18, 2014. Demands are payments made by the County. Specific details of the demands may be requested through the County public record request process.

*C.  District I: Request approval of a letter of support for the Housing for Heroes Project.  

5.  County Manager's Office: Presentation and possible discussion regarding the projected State Budget. 

6.  Election Department: Pursuant to A.R.S. 11-424(E), determine by lot the final position for the Vernon Elementary School District No. 9, to be determined between Irene M. Humphrey and John Wilhelm, both of whom received an equal number of votes.

7.  Election Department: Discussion and possible approval of the Canvass of the November 4, 2014 General Election.

8.  Sheriff's Office: Discussion and possible approval to sell a 2000 Chevrolet C2500 van to the St. Johns Emergency Services for use in the Explorer Training program. This youth program benefits Fire, EMT, Law Enforcement and the community as well as the youth that participate through mentoring and civic development. The price for this sale would be $100.00 pursuant to the Board's authority under A.R.S. 11-251. The van is a former detention vehicle that is being retired from Sheriff's Office use.

9.  County Manager's Office: Discussion and possible approval of a request to implement a wage adjustment for certified law enforcement officers working within the Sheriff's Office.

10.  Engineering Department: Discussions and possible approval to award Bid #201504 for blasting services at the County's limestone pit.
 
11.  Erin Collins, AZGEBT: Update on the County's health insurance and the Sweet Savings Diabetes Program.

12.  County Manager's Office: Possible Executive Session pursuant to A.R.S. 38-431.03(A)(3) for legal advice due to an employee issue related to the Primary election.

13.  Notification of Small Counties Forum meeting on November 19, 2014 at 6:30 p.m. at the Hilton Garden Inn Phoenix Airport North, located at 3838 East Van Buren Street in Phoenix and the County Supervisors Association meeting (CSA) on November 20, 2014 at 10:00 a.m. to be held at the CSA building, 1905 W. Washington Street in Phoenix where two or more members of the Apache County Board of Supervisors may be in attendance.

14.  Call to the Public: Individuals may address the Board on any relevant issue for an amount of time determined by the Chairman. At the close of the call to the public, Board members may not respond to any comments but may respond to criticism, ask staff to review a matter, or ask that a matter be placed on a future agenda.
